Template repository

Create a template repo on GitHub that has all the content we need when we start a new microlib microservice.

This has two purposes:

1. make it easier/faster to start a new microlib microservice
2. make all of our microservices relatively similar as far as structure and documentation goes (even when they're built with different technologies)

@dimitraz to begin looking at our current repositories and list the things that should be within each repository, and how they should be structured.
